Article Designing a Learner-Centered ESP Course for Adults Based on Evidence from a Questionnaire and Incorporating the Learners' Aims into a Situational-Based Syllabus(Ankara Üniversitesi TÖMER, 2010) Dinçay, TurgayThis paper examines the organization of a six-week Conversational English ESP course design for the tweny-two adult administrative human resources staff working in the International Inlingua Language Learning Center in Istanbul. At the very start of this course, a thorough needs analysis taking into account the language needs of the learners based on the essentials put forth by Hutchinson and Waters (1987) and Dudley-Evans and St.John (1998) established the core framework for the study and the findings from this analysis were used for the ESP course and syllabus design. In the final phase of my study for the ESP course design in question, the choice of the right syllabus was a major decision in language teaching, and it was made as consciously and with as much information as possible. The different inhalable particles were collected by using a cascade impactor in eight size fractions (<0.4 mu m, 0.4-0.7 mm, 1.1-2.1 mu m, 2.1-3.3 mu m, 3.3-4.7 mu m, 4.7-5.8 mu m, 5.8-9 mu m and >9 mu m) for six months at each station. Samples were collected on glass fiber filters and filters were extracted and analyzed using ICP-MS. Log-normal distributions showed that the particles collected at the Yenibosna site have a smaller size compared to the Goztepe samples and the size distribution of PM was represented the best by the tri-modal. The average total particle concentrations and standard deviations were obtained as 67.7 +/- 17.0 mu g m(-3) and 82.1 +/- 21.2 mu g m(-3), at the Yenibosna and Goztepe sites, respectively. The higher metal rate in fine and medium coarse PM showed that the anthropogenic sources were the most significant pollutant source. An experimental setup was built for this purpose. 105 motor units were measured. 44 of them belonged to JME group consisting of 9 patients and 61 of them belonged to NC group comprising ten healthy volunteers. k-fold cross validation was applied to train and test the models. ROC curves were drawn for k values of 4, 6, 8 and 10. 100% of detection sensitivity was obtained for DT, NN, and NB classification methods. Bone quality is poor in these patients. The purpose of this study is to evaluate the feasibility of fusion and accuracy of fluoroscopy in cervical transpedicular fixation (CPF) in a standardized clinical protocol for RA patients. Materials and Methods: 96 pedicles of 15 patients operated between January 2000 and ay 2010 due to atlanto-axial and subaxial cervical subluxation were investigated for post-operative malposition of the transpedicular screws. Three-dimensional computed tomography was used as a useful tool in preoperative planning and in transarticular or transpedicular screw placement with the free hand technique in the cervical spine of RA patients. Fixation and reduction with fusion was performed in all of the patients, and autogrefts from iliac wing were used for fusion. Ranawats and Nurick scales were used to assess the results. All screws were evaluated by Kasts criteria. Fusion or stability was evaluated on plain radiographs taken 3 weeks and 6, 12 and 60 months after the surgery. Results: Female to male ratio was 6/9. The mean age at the time of surgery was 57.4 years (range 44-72 years). Five of the patients were operated for both C1-2 and subaxial subluxation. Two of the 15 patients had only C1-2 subluxation and the remaining eight patients had only subaxial cervical subluxation. The screws were at their correct places in 84 pedicles (87.5.%) while minor breach was detected in 9 (10.9%). According to Ranawats criteria, seven patients remained the same, and eight patients showed improvement. Instrumentation failure, loss of reduction or non-union was not observed at the final follow-up (average 31.5 months; range 24-60 months). Conclusion: CPF provides a very strong three column stabilization and solid fusion in the osteoporotic vertebrae, but also carries a risk of vascular injury without nerve damage or in the RA patients, but the risk is low in experienced hands.Article Citation - WoS: 9Citation - Scopus: 15The Molecular Genetics of von Willebrand Disease(Galenos Yayıncılık, 2012) Berber, Ergül; Berber, ErgulQuantitative and/or qualitative deficiency of von Willebrand factor (vWF) is associated with the most common inherited bleeding disease von Willebrand disease (vWD). vWD is a complex disease with clinical and genetic heterogeneity. Incomplete penetrance and variable expression due to genetic and environmental factors contribute to its complexity. vWD also has a complex molecular pathogenesis. Some vWF gene mutations are associated with the affected vWF biosynthesis and multimerization, whereas others are associated with increased clearance and functional impairment. Moreover, in addition to a particular mutation, type O blood may result in the more severe phenotype. Childhood absence epilepsy and juvenile absence epilepsy are among the most common subtypes of IAEs. While the role of ion channels has been the primary focus of epilepsy research, the analysis of mutation and association in both patients with absence epilepsies and animal models revealed the involvement of GABA receptors and calcium channels, but also of novel non-ion channel proteins in inducing spike wave discharges (SWD). Functional studies on a mutated variant of these proteins also support their role ill the epileptogenesis of absence seizures. Studies in animal models point to both the thalamus and cortex as the origin of SWDs: the abnormalities in the components of these circuits leading to seizure activity. This review examines the current research on mutations and susceptibility alleles determined in the genes that code for the subunits of GABA receptors (GABRG2, GABRA1, GABRB3, GABRA5, GABA((B1)) and GABA((B2))), calcium channels (CACNA1A, CACNA1G, CACNA1H, CACNA11, CACNAB4, CACNAG2 and CACNG3), and novel non-ion channel proteins, taking into account the results of functional studies on these variants. Studies have shown that the expression of upregulated gene 4 (URG4), which promotes cell growth and survival, is increased in different types of carcinomas including hepatocellular carcinoma, gastric cancer and osteosarcoma. Similarly, higher expression of URG4 and cyclin D1 gene might promote proliferation of the blast cells by causing escape from the G1 checkpoint and entry into the S phase. This study reports the high expression level of URG4 in 2 high-risk ALL patients for the first time in the literature. In conclusion, the higher expression of URG4 in our 2 patients suggests that URG4 might be involved in leukemogenesis. Future studies with a large number of high-risk ALL patients and cell culture studies are needed to demonstrate the exact role of URG4 in leukemogenesis. Copyright (C) 2012 S. Although previous research gave clear evidence that language is closely related to depression, the research on intervening variables in the relationship has been limited. The present investigation tested a structural equation model in which self-concept clarity and self-consciousness mediated the relationship between personal perceptions of language and depression. Since the need for absolute truth construct has been shown to be important in providing greater consistency in estimates of the relationships among the variables, it has been added to the model as a control variable. The results supported the model and showed that personal perceptions of language predicted self-concept clarity, which in turn predicted the participants self-reflection and self-rumination. This calculation is performed using volume data through the mode Gruneisen parameter. It is found that our calculated frequencies of those lattice modes increase with increasing pressure, as expected. HandeAn increasing number of mutations and polymorphisms have been identified in the von Willebrand factor (VWF) gene of patients with von Willebrand disease (VWD). Most of the sequence alterations are within exon 28, duplicated in the VWF pseudogene on chromosome 22. Genetic recombination causing the gene conversion between the VWF gene and its pseudogene is associated with multiple substitutions in the VWF gene and with VWD. In the present study, VWF gene exon 28 was analyzed in 33 patients with VWD by DNA sequencing. A total of 73% of the patients were heterozygous for p.D1472H, p.V1485L, p.1500A, p.1501F, p.L1503P, and p.S1506L single-nucleotide polymorphisms. Family analysis revealed that the gene conversion occurred between the VWF gene and its pseudogene in 3 patients. Case-control association analysis by Haploview 4.2 did not show an association between the haplotype and VWD. The aim of the present study was to illuminate the association between language use (operationalized by the gap between experience and languageGAP) and mental health (e.g., adjustment and depression, with the mediator role of emotion regulation). Three-hundred-and-seventy-four adolescents (220 female, 154 male; 16.08 mean age) participated in the study. The results provided support for the mediator role of emotion regulation in the relationship between GAP and mental health. The role of beta oscillations upon cognitive stimulation is least studied in comparison to other frequency bands. The study included 17 consecutive patients with MCI (mean age = 70.8 +/- 5.6 years) according to Petersen's criteria, and 17 age- and education-matched normal elderly controls (mean age = 68.5 +/- 5.5 years). The experiments used a visual oddball paradigm. EEG was recorded at 30 cortical locations. EEG-evoked power, inter-trial phase synchronization, and event-related beta responses filtered in 15-20 Hz were obtained in response to target and non-target stimuli for both groups of subjects. In healthy subjects, EEG-evoked beta power, inter-trial phase synchronization of beta responses and event-related filtered beta responses were significantly higher in responses to target than non-target stimuli (p < 0.05). In MCI patients, there were no differences in evoked beta power between target and non-target stimuli. Furthermore, upon presentation of visual oddball paradigm, occipital electrodes depict higher beta response in comparison to other electrode sites. The increased beta response upon presentation of target stimuli in healthy subjects implies that beta oscillations could shift the system to an attention state, and had important function in cognitive activity. This may, in future, open the way to consider beta activity as an important operator in brain cognitive processes. (C) 2013 The Authors. This research addresses this issue by taking into consideration two important suppressor variables, self-rumination and the need for absolute truth. The latter is an innovative variable, defined and presented in this research. The first two studies aimed to validate a new measure that acts as an operational definition of the need for absolute truth. The first study was conducted with two group of participants; the first group consisted of 129 females and 67 males, mean age=20 years, and the second 182 females and 104 males, mean age=27. In the second study, participants were 22 females and 18 males, mean age=20.5. In the final study, conducted with 296 female, 163 male participants (mean age=37), suppressor effects were tested using structural equation modeling. The results showed that by taking account of these two suppressor variables, particularly the need for absolute truth, the expected relationship between self-reflection and mental health was revealed. Event-related oscillations might be used for detection of cognitive deficits. Our group's earlier results showed diminished delta visual and auditory target oscillatory responses in AD, and we investigated whether this prevails for MCI. Eighteen MCI subjects and 18 age-matched healthy elderly controls were investigated. The maximum peak-to-peak amplitudes of oscillatory responses for each subject's averaged oscillatory target responses in delta, theta, and alpha frequency bands upon application of visual oddball paradigm were measured. Repeated measures of ANOVA was used to analyze four locations (frontal, central, parietal, occipital), at three coronal (left, midline, right) sites. Independent t tests were applied for post-hoc analyses. The oddball target delta response (0.5-3.0 Hz) was 26-32% lower in MCI than healthy controls over fronto-central-parietal regions [F(1.34) = 4.562, p = 0.04]. Without a group effect, theta oscillatory responses (4-7 Hz) showed significant differences in coronal electrodes indicating highest values over mid-electrode sites, and a anteriorposterior x coronal effect, being maximum at mid-central. Alpha frequency band analyses indicated no statistical differences. Peak-to-peak amplitudes of visual target delta oscillatory responses were lower in fronto-central-parietal regions in MCI than in healthy controls. This supports our earlier findings in AD, showing hypoactive delta fronto-central-parietal regions during cognitive tasks. This single-blinded randomized trial was performed at Istanbul University Institute of Oncology between October 2010 and January 2011. The patients, diagnosed with breast, gynecological, or lung cancer treated by doxorubicin-based- or cisplatin-based treatment were included in the study. Taking the consent of the patients, they were divided into experimental (n = 67) and control groups (n = 53) in line with the randomization list prepared before. The patient description form, Rhodes Index of Nausea, Vomiting, and Retching (INVR), and Functional Assessment of Cancer Therapy-General were used in data collection. Non-parametric tests were applied. The patients in the experimental and control groups were identical in terms of sociodemographic features and their conditions. Most of the patients were married (84 %), and some were primary school graduates (41.7 %) and had mid-level income (54.2 %). In nausea-vomiting-retching, experiences of the patients before using a placebo and a real acupressure were identical and similar changes had been observed during 5 days after the treatment. It has been determined that there was no difference between the groups statistically. A similar result has been observed when the impact of acupressure on the subgroups of sociodemographic features and condition were examined, and it has been concluded that real acupressure application cannot increase the quality of life. It has been determined that CINV is directly related to the treatment, and acupressure wristband was not an effective approach in preventing CINV.Article Citation - WoS: 2Citation - Scopus: 2An Unexpected Complication of Occipitocervical Stabilization Surgery: Retropharyngeal Hematoma(Elsevier Science Inc, 2013) Kotil, Kadir; Koksal, Neslihan Sutpideler; Ozyuvaci, Emine; Yigit, Ozgur; Kilic, KayaBACKGROUND CONTEXT: To report a unique case of an unexpected complication of occipitocervical stabilization surgery that is retropharyngeal hematoma (RH). PURPOSE: Postoperative RH is a very rare complication and has never been reported after posterior occipitocervical surgery. STUDY DESIGN: Case report. METHODS: A 44-year-old woman being treated for rheumatoid arthritis for 20 years was admitted to our hospital in a wheelchair with the complaints of neck pain and weakness in both upper and lower extremities. She was diagnosed with basilar invagination, and occipitocervical (C0 C5) transpedicular fixation with osteosynthesis using iliac autograft was performed. The airway was seen as obstructed after extubation. The airway was maintained with laryngeal mask, and computed tomography revealed an RH. Emergent tracheotomy was performed. The patient was decannulated because of the resorption of RH after 10 days and was discharged. CONCLUSION: This patient is the first patient, to our knowledge, to be reported for unexplained RH after cervical posterior spinal surgery. (C) 2013 Elsevier Inc. Hamza-nâmelerin önce sözlü bir gelenek olarak Türkler arasında itibar gördüğü anlaşılmaktadır. Hamza-nâme’nin Türk halkı tarafından sevilmesinde Hazreti Hamza’nın cesareti, dürüstlüğü ve daima zayıftan yana olması gibi sebepler etkili olmuştur. Hamzanâme 72 kitaptan oluşmaktadır. İstanbul Üniversitesi Edebiyat Fakültesi Nadir Eserler Kütüphanesinde bulunan nüsha, nitelik ve nicelik bakımından farklı önemi haizdir. Destansı ögelerin kimi ciltlerde daha yoğunlukta olduğunu gördüğümüz bu muazzam eser, yazıldığı dönemden itibaren çok geniş bir coğrafyada farklı zümreler tarafından okunmuştur. Çok ilginç bir okuma tarzının varlığına şahit olduğumuz Hamza-nâme ciltlerinin hemen hemen 72’sinde de çok sayıda kayda geçirilmiş “okundu” bilgileri ile karşılaşılır. Bu kayıtlarda; eserin hangi cildi okunmuşsa, okumanın ardından, farklı kalemle eseri okuyanın adı, cildin okunduğu mekânın adresi, okuyanların ve hatta bazen dinleyenlerin kısa tanıtımı yer almaktadır.
